Amlodipine besilate
Indications/Uses
1st-line treatment of HTN as a monotherapy to control BP. Reduces the risk of fatal CHD, non-fatal MI, stroke, coronary revascularization & hospitalization due to angina in CAD patients. 1st-line treatment of myocardial ischemia due to stable angina &/or Prinzmetals's or variant angina of coronary vasculature. In combination or as monotherapy for angina ie, refractory to nitrates &/or β-blockers.
Dosage/Direction for Use
HTN & angina Initially 5 mg once daily. Max: 10 mg. CAD 5-10 mg once daily. Childn 6-17 yr HTN 2.5-5 mg once daily.
Administration
May be taken with or without food: Do not take w/ grapefruit or grapefruit juice.
Contraindications
Hypersensitivity to amlodipine & dihydropyridines.
Special Precautions
Increased risk of pulmonary edema in patients w/ NYHA III & IV heart failure of non-ischemic etiology. Impaired hepatic function. Pregnancy & lactation.
Adverse Reactions
Headache, dizziness, somnolence; palpitations; flushing; abdominal pain, nausea; edema, fatigue.
Drug Interactions
Increased systemic exposure w/ simvastatin. Increased plasma conc w/ strong CYP3A4 inhibitors (eg, ketoconazole, itraconazole, ritonavir). Increased BP-lowering effects w/ grapefruit juice. Increased risk of hypotension w/ clarithromycin. Decreased plasma conc w/ CYP3A4 inducers eg, rifampicin, Hypericum perforatum. Risk of increased blood levels of tacrolimus.
